Dec 24: [video]  Residents from a dozen homes are forced to evacuate Christmas Eve due to an explosion and fire at a Williams Partners L.P. Meter Station in Chartiers Township, Washington County, PA.
Dec 24:
The families that were evacuated due to the massive gas leak at the Triad Hunter pad in Monroe County, OH are allowed to return to their home.
Dec 23: The PA DEP
responds to an anonymous complaint in reference to a fire on CNX's Crum NV 55 AHS in Washington County, PA. CNX did not report the fire to the department
although serious damage occurred.
Dec 23:
Colorado-based Vantage Energy is forced to pay nearly $1 million in fines for damage to streams that was caused by a landslide and wastewater spill at a well pad in Greene County, PA
Dec 22: XTO is fined $2.3 million for violations of Section 404 of the Clean Water Act and will spend an estimated $3 million to restore eight sites damaged by unauthorized discharges of fill material into streams and wetlands in connection with hydraulic fracturing operations in WV.
Dec 22:
Tennessee Gas Pipeline Company is fined around $800.000 for violations in
Pike, Potter, Susquehanna, and Wayne Counties in PA.  The violations include discharge of sediment into protected waterways.

) after an explosion at a rig near Colgate, OK.
Dec 17: [video] A crew called in from TX is unable to control the gas leak at the Triad Hunter well pad in Monroe County, OH.  Families are told to expect to be out of their homes for at least another 7 days.
Dec 15: Rex Energy (Re Gas Dev) is cited by the PA DEP for SWMA301 - Failure to properly store, transport, process or dispose of a residual waste at a site in Butler County, PA.
Dec 14: [video] 30 homes are evacuated within a mile and an half after a gas leak at the Triad Hunter pad in Monroe County, OH.
Dec 11: Contractors working in the Marcellus Shale are forced to pay nearly $4.5 million in back pay to 5,310 workers in Pennsylvania and West Virginia after an investigation that found violations of the federal Fair Labor Standards Act.
Dec 11: David Nesbitt, 51, of Wilkes-Barre, was killed after his water tanker slid on ice in Susquehanna County, PA.
Dec 11: XTO is ordered by the DEP to cease and desist in well construction on the Cratty Unit, in Butler County PA, after they were found to operating without the proper permits.
Dec 11:  [video] A large gas leak at a Comstock Resources well in Burleson County, TX causes the evacuation of 25-30 homes within a 2-mile radius.  It leaked gas at the rate of 2,700 cubic feet per hour
Dec 5: Wpx Energy Appalachia is cited by the DEP for OGA 3218(A) - Failure to restore or replace a public or private water supply affected by a well operator in Westmoreland County, PA
Dec 4: Markwest is caught operating a compressor station in Westmoreland County, PA without the proper permits.
Dec 4: Chesapeake is cited by the DEP for 78.85 - Inadequate, insufficient, and/or improperly installed cement, 78.73B - Excessive casing seat pressure, and OGA 3217(B) - Failure to prevent migration of gas or fluids into sources of fresh water causing pollution or diminution. Failure to properly case and cement well through a fresh water-bearing strata in regulated manner or depth in Bradford County, PA.
Dec 3: The DEP fines Cabot $120,00 for an explosion at a well site in Susquehanna County, PA that caused around 2,835 gallons of wastewater to spill, some escaping containment and reaching nearby soil, according to the DEP.  One worker suffered burns.
Dec 3: Trimont Energy is cited by the DEP for 78.85 - Inadequate, insufficient, and/or improperly installed cement at a well in Venango County, PA.
Dec 1: A Regency Energy compressor station explodes around 4 am in Susquehanna County, PA

April 30-A CSX train carrying fracked crude oil derails and bursts into flames in downtown Lynchburg, VA, spilling oil into the James River and forcing hundreds to evacuate. [video]
April 30- Two oil workers die and nine others are injured in an explosion at a RKI Exploration and Production well site in West Texas. Loving County Sheriff Billy Hopper says the workers were operating a rig that separates production fluids, such as oil from water.
April 27- Zachary Buckles, 20, of Glasgow, Mont, dies at a Continental well site 5 miles south of Williston, ND of apparent H2S gas exposure. This is the 38th oilfield death investigated by OSHA since fiscal year 2010.
April 25-Eight families sue a natural gas company for personal injuries they attribute to fracking operations in AK.
April 25-Four people are injured, due to a lightning strike a Southwestern Energy gas drilling site in Faulkner County, AK.
April 23-Around 20 garbage cans full of illegally dumped oil filter socks are been found north of Crosby, ND. 
April 23-An explosion at a Williams natural gas processing plant Opal, Wyo. causes the evacuation of the town.  
April 22-A six-person Dallas jury awards a middle-class north Texas ranching family nearly $3 million from Aruba Petroleum Inc. company whose drilling caused years of sickness, killed pets and livestock, and forced them out of their home for months.
April 18-500 tons of soil is removed after a “significant leak” is discovered at a frack pond operated by Range Resources.  There are questions as to whether or the DEP was properly notified. 
April 10-Chevron is cited with nine violations for their February gas well explosion in Greene County, PA including one count of refusing to allow state investigators onto the property, in violation of oil and gas laws.
April 9-High pressure causes approximately 650 barrels of oil and 450 barrels of brine to spill from a tank on an Oasis Petroleum site in ND.
April 9-A substance used to give natural gas its odor leaks from a dehydration station in Wyoming County, PA. [video]
April 7-A 37-year-old worker is found dead at a Noble Energy pump station in WV.
April 7-An illegally dumped “frack sock” (a filter used in the process that can be radioactive) is discovered  along the road in Dunn County, ND. [video]
April 5-Six families are evacuated after a 12-inch pipeline operated by Williams explodes. The cause of the explosion and fire is thought to be shifting earth. [video]
April 5- David Zamora, 43 from Victoria Texas, is crushed to death by a piece of heavy equipment at the Rice Energy Big Foot Pad Smith Township, Belmont County, OH.
April 4- A compressed natural gas (CNG) vehicle explosion kills 59-year-old Gerald Throw, of Sobieski, Wis. and injures passenger, 29-year-old Jose Perez of Green Bay. Sheriff's officials say a box-style truck was slowing to make a turn into a parking lot of Ace Manufacturing Inc. when its load shifted, potentially compromising the truck's compressed natural gas fuel system. That fuel system detonated.
April 2-A semi-truck tips over in Marshall County, WV spilling hazardous material on the road. The road is closed for nearly 12 hours.  Reports indicate several drums containing poisonous liquid spilled onto the road. A HAZMAT crew was called to work to clean up the mess. The driver of the truck was not endorsed to carry this kind of dangerous material and citations are pending, according to deputies.
April 2- True Oil Llc is cited by the DEP for violation of 78.55 - No Control and Disposal/PPC plan or failure to implement preparedness, prevention, and contingency (PPC) plan in West Mahoning Twp., Indiana County, PA.  The PPC plan involves the transfer of drilling waste.
April 1- Atlas Resources Llc spills and “unknown amount” of brine that is carried out of the secondary containment because of “[r]ecent precipitation events.”  A vegetation kill is noted.

Feb 26-A gas well blowout in Acadia, LA has forced the evacuation of 40 people within 1 ½ mile of the well.  It will several days before the well is capped. [video]

-Cabot is cited with “Operator shall prevent gas and other fluids from lower formations from entering fresh groundwater. Failure to operate and construct the well to ensure the integrity of the well is maintained and health, safety, environment, and property are protected” in Dimock, PA.

-XTO is cited for “Improper casing to protect fresh groundwater Inadequate number of centralizers installed on surface casing. Spacing was 200', whereas regulations dictate that spacing should be no more than 150' apart” at a well in Forward Twp., Butler County, PA.

Jan 31-A Canadian National Railway Co train carrying fuel oil and other hazardous materials derails and spills in southeast Mississippi, forcing the evacuation of nearby residents.
Jan 31-A Tulsa, Okla., is fined $1 million for dumping 4,700 gallons of crude oil into a tributary of the North Platte River.  An independent contractor was regularly draining produced water into the creek. Company officials were aware of the discharges, but did not report them to the authorities, the investigation found.

Nov 29-A truck carrying water from a gas-drilling site flipped into a creek near the Dallas Pike Methodist Church in Ohio County, WV. Officials said that the driver was taken to a hospital for injuries.
Nov 29-30-inch pipeline ruptured causing an explosion near Hughesville, MO.  The fireball could be seen for 30 miles.  Three homes were evacuated. [video]
Nov 25-A pipeline leak in Montana released 17,000 barrels of brine, polluting more than a mile of a creek in the Badlands of southwest North Dakota.
Nov 14
-A gas line explodes half-a-mile from an Ellis County school. A child in the school said, “We were all crying in my class.”  Families are evacuated from a 1 ½- 3 mile radius.  They will be out of their homes and businesses for 24-36 hours. [video]

Late Sept- More than 200 barrels of fracking fluid, oil and water blew out of a traditional oil well in NM due to pressure from a nearby Encana fracking operation.  The fluid created a stream of liquid that reached a half-mile in length and crossed the mail road. [video]

Aug 31- Kevin Figaniak, a 21-year-old Wheeling Jesuit University student, was beaten to death by Craig Tyler Peacock and Jarrett Mathis Chandler.  Peacock and Chandler are from out of the area and are reported to be working in the natural gas industry.
Aug 30- A Chevron pipeline leaked an unknown quantity of natural gas in the Gulf of Mexico near Pascagoula, Mississippi.
Aug 29- Jesse A. Grimsley, 31, of Shevlin, Minn, was killed in a Sheehan Pipeline Construction accident near Steubenville, OH.  According to the sheriff’s office, Grimsley had filed a daily equipment check noting "brakes will not hold on hill.”  Sheehan Pipeline Construction is based in Tulsa, Okla.
Aug 29- Michael Guesman of Cortland admits in U.S. District Court in Cleveland that his former boss at Hardrock Excavating, Ben Lupo ordered him to dump brine waste on 24 separate occasions into the Youngstown sewer system.
Aug 29-An oil rig exploded in Lavaca County, TX, triggering a massive fire [video]
Aug 27-Caller discovers tanker truck dumping brine water on the roadway and running into Clearfork Creek near Grayville, OH
Aug 23-A breached frack pond operated by Berry Petroleum flooded nearby residential property in Gardendale, TX.
Aug 22-Residents in Chartiers Twp., PA complained of burning eyes, throat nausea and a metallic taste their mouths after another flaring incident at MarkWest’s natural gas processing plant.  This is at least the third flaring incident at the plant in the past six weeks.
Aug 22-A pipe manufacturer in the Marcellus region spilled 5,000 gallons of sulfuric acid into the Shenango River.  
Aug 21-Chesapeake Appalachia is cited with not reporting two pollution incidents in Beaver County, PA.  Soil samples and eye-witness accounts suggest petroleum products and flowback wastewater spilled onto the ground. 
Aug 20-A 16-inch natural gas pipeline exploded in OK sending flames 200 ft. in the air and damaging a barn.
Aug 20-A semi-truck hauling for oil and gas wells in the area struck and killed 57-year-old Dan Wilson of Weirton, WV along U.S. 22.  The truck company was out of Indiana. [video]
Aug 20-West Virginia environmental regulators cited MarkWest Energy for causing a fishkill in Rocky Run, a tributary of Fish Creek after one of their pipelines ruptured.  Minnows, smallmouth bass and other species of fish died from the spill.
Aug 16-A Texas longhorn steer was killed by exposure to poisonous hydrogen sulphide (H2S) gas from a pipeline leak in Alberta.
Aug 16- An Antero Resources site in Harrison County, WV caught ablaze sending three workers to the hospital, two via airlift.
Aug 16- Records were released indicating that OSHA cited Central Environmental Services LLC for the death of Brian Hopkins caused by an explosion at an EQT Corp. well pad in Taylor County, WV.
Aug 13-A landslide ruptured a MarkWest Energy pipeline in Wetzel County, WV causing a leak of an unidentified, but potentially explosive, liquid into a nearby stream.
Aug 13-Around 80 families were evacuated when a pipeline carrying gas byproducts ethane and propane somehow ruptured, caught fire, and exploded in IL.
Aug 10-A 24-year old was killed when a block from a New Star Energy fell and crushed him near Highvale, Alberta. [video]
Aug 7- A joint study from the U.S. Geological Survey and the U.S. Fish and Wildlife Service found that a Nami Resources Company’s fracking fluid spill in Kentucky in 2007 likely caused the widespread death of several types of fish. 
Aug 6-A driver was cleared of any wrongdoing  for failing to stop after his 18-wheeler carrying fracking wastewater crushed a 14-year-old boy in Fort Worth, TX on April 24, 2013.  Deston Bibbs died as a result of his injuries.
Aug 3-In Tioga County, PA, Swepi Lp spilled 5 barrels of produced water from hole in the coil tubing unit onto the containment and pad. Holes were discovered in the containment liner.
Aug 1-A truck hauling brine collided with two cars in Vienna Township, OH.  Injuries were reported.  
Aug 1-In Lawrence County, PA, a Rockwater Energy Solutions truck carrying ethylene glycol to nearby fracking operations crashed spilling 265 gallons of the toxin poison in a field.  Unfamiliarity with the road was cited as one of the causes of the accident.
Aug 1-Two families in TX file a class-action lawsuit claiming that fracking caused damage to their real estate and homes.

June 26-Two county workers suffered minor injuries and at least 34 cars were damaged when a section of roadway sunk as a result of a Sunoco Pipeline LP project in Beaver County.
July 25- In Susquehanna County, PA, Chief Oil and Gas spilled an estimated 20 barrels of oil due to a valve that was left open.  It then leaked onto the ground through a hole in the plastic lining.  
June 25-A caller to the NRC reported a fire at an oil well near Ravenna, OH.  There were 150 gallons of oil released.
June 25-A 30-inch transmission pipeline exploded in Enon, LA causing the evacuation of 55 people.
June 22-A water tuck owned by JB Oil and Gas ran a stop sign killing a mother and her 14 year old daughter. 
June 21-PVR’s Chapin Glycol Dehydration plant in Monroe Twp accidentally discharged in the atmosphere.  This is the 3rd such event since the PVR Chapin plant went operational in late spring of 2012. [video] 
June 21 -32-year-old Greg Peacock, died from 3rd degree burns he suffered during an explosion at a well pad.  He left behind a two year-old son and fiancee.
June 20-50 homes were evacuated when a gas pipeline containing 1% deadly hydrogen sulfide ruptured southwest of Calgary after debris from a flood struck the pipeline.   
June 18-A 30-inch transmission line in Louisiana ruptured, sparking a natural gas explosion and a fire.
June 13- A gas line ruptured underneath the Ohio River near Bellaire spewing gas out of the water between OH and WV [video]
June 12 -A truck at a well in MI backed into a high-pressure line causing an explosion that knocked people over in a nearby home.  
June 12 -A Williams cracker plant in LA exploded killing 2 and injuring 100.  This is the same type of plant that is proposed for Beaver County.
June 7-Investigations determined that Vintage Oil illegally dumped formation fluids and fracking fluids into an unlined pit in CA [video]
June 4- PA DEP fined PVR Marcellus Gas Gathering LLC of Williamsport, Lycoming County, $150,000 for multiple and continuing violations of the Pennsylvania Clean Streams Law, the Dam Safety and Encroachments Act and various related environmental regulations, including sediment discharges into High Quality and Exceptional Value streams during construction of the Coal Mountain pipeline in four Lycoming County townships
June 4-Wastewate hauler Harch was ordered to suspend operations in OH after evidence was found that the company was illegally disposing waste in a private pond [audio]
June 3- SWEPI LP was cited in for “failure to properly store, transport, process or dispose of a residual waste” at their West 1 OG Well in Slippery Rock Twp.
June 3- RE Gas Dev LLC (Rex Energy) was cited for “discharge of industrial waste to waters of Commonwealth without a permit” at their Warner Unit 1H in Lancaster Twp.
June 1-A pipeline operated by a Texas-based oil company has leaked 9.5 million litres of industrial waste water in Canada.
June 1-A caller to the NRC reported that a water tanker truck was dumping into Indian Creek in Tyler County, WV.

April 30- 9,000 gallons of wastewater spilled onto a miniature horse farm and into the farmhouse basement and garage from a Carrizo well site.  Fluid spilled from another Carrizo well site in Wyoming County in March [video]
April 27-A considerable amount of natural gas and crude oil residue blew out of an Atlas Energy pipeline Lafayette Twp., Bradford County.
April 27-More than 100 barrels of oil-based drilling mud spilled into Cadron Creek after a truck accident in WV.
April 17- MarkWest natural gas operations in Butler and Washington Counties were cited by the EPA for being in violation of federal clean air laws.
April 11-Two men died after an explosion at Eureka Hunter Pipeline operations near Wick, W.Va
April 4-An explosion at a natural gas compressor station in Logan County, OK forced the evacuation of homes within one square mile of the incident.
